Assistant Manager / Manager, Primary Care Transformation (Healthcare)

 JOB DESCRIPTION
  • Supporting the primary care transformation efforts.
  • These include establishing collaborations with various healthcare stakeholders such as hospitals, general practitioners, and community and social care providers.
  • Support the development of a new care model in the polyclinics. You will also be responsible for working closely with various other departments in designing patient-centric workflows and managing projects that positively impact patients’ health outcomes. This includes conceptualizing, planning and executing care coordination and integration initiatives within the cluster and with other health and social care partners.
  • Relationship building with relevant partners, along with being conversant of the healthcare needs of our patients across the care continuum.
 
 
Requirement
  • Degree in any discipline with at least 5 - 7 years of working experience with minimum 3 years of supervisory experience.
  • Candidates with lesser years of experience will be considered for Senior/ Executive position.
  • Pleasant disposition and personality
  • Good decision making and problem solving skills with an analytical mind
  • Possess excellent interpersonal and communication skills
  • Knowledge of LEAN is a plus
  • Required skill(s): Healthcare, Collaboration, Planning, Integration Initiatives.
